10/29/24
Effective January 1, 2025, Dr. Uthman Cavallo will be dramatically decreasing one-on-one patient care and PHA as a whole will be moving in a new direction.
Both Drs. Cavallo will be focusing on group & community care.
We will be holding a two-part “Celebration of Impact” Event on Saturday, January 11, 2025 for both Dr. Uthman Cavallo and Dr. Susan Hart-Cavallo (who made this difficult decision herself a few years ago).
This event will give you the opportunity to:

Hear directly from the Cavallos about the genesis of this transition and their vision for the season ahead

Chat with them and process what this change means for you

Hear from others about how both Drs. Cavallo have impacted their lives and health journeys

Share your own story if you feel so inclined
Part 1 will be a virtual gathering held via Zoom and Part 2 will be in-person open house later that day (venue TBD).

So, what does this new chapter mean for PHA?
MAJOR CHANGES EFFECTIVE 1/1/25
#1: Our primary focus will no longer be on customized individual medical management, but on our group-based membership community
Dr. Uthman Cavallo will be shifting most of his time and energy from providing medical care to cultivating and co-leading our new Acts2Thrive (A2T) Community alongside PHA co-founder, Dr. Susan Hart-Cavallo.
As Dr. Cavallo has expressed to many of you, he had been envisioning such a career pivot much further down the road—a couple years at least—after beginning with the reduction in pregnancy care described in last month’s announcement.
However, since then, it has become abundantly clear that his current schedule is getting in the way of all that God is calling Dr. Susan and him to do.
Acts2Thrive Membership Enrollment will open by the end of November. It will remain open through January 15th, at which time enrollment will close until we have determined that we have the capacity to accept more members.

#2: We are restructuring & limiting one-on-one medical services
Restructuring: Patients of Dr. Cavallo who decide to continue on at PHA will be seen instead by Nurse Practitioner Sara Arbogast with whom he collaborates. However, Dr. Cavallo will continue to perform surgeries and will be engaging with members regularly in group settings.
Limited Access: We will be adjusting the structure of provider schedules and this will result in a drastic reduction in the number of patients for whom we are able to provide one-on-one medical management.
Therefore our Acts2Thrive (A2T) Community Members will have exclusive access to treatment and ongoing medical management, as slots are available.
However, non-members will still be able to get comprehensive, holistic work-ups for pelvic pain, irregular periods, endometriosis, polycystic ovarian syndrome, and infertility.

#3: We will no longer bill insurance
As part of this transition, we will be terminating all insurance contracts. Insurance companies do not compensate fairly for the work that we do and in some cases do not pay at all.
All medical care provided to both members and non-members will be fee-for-service.
We will provide a detailed invoice for you to submit to your insurance yourself for possible reimbursement (depending on your out-of-network benefits).

IN THE INTERIM
Surgery Scheduling:
If you are in need of surgery and want to have it covered by your insurance, we are working to carve out additional time for surgery in November and December.
If you are waiting to schedule a surgery OR if you were previously undecided but now would like to proceed with surgery, please give our office a call ASAP and we will do our best to make it happen before the termination of our insurance contracts.
Your Upcoming Appointment in 2024:
In these last few months of the year, there will be many adjustments to provider schedules.
** Keep a close eye on all communication from us moving forward as we may need to change or cancel your scheduled appointment.**
Appointments & Surgeries Scheduled in 2025:
We are canceling all 2025 appointments and surgeries. We will be starting fresh with all provider schedules. If you have a surgery already scheduled in 2025, give our office a call ASAP and we will do our best to work you in on a first-come, first-served basis.

We understand that these are sudden and significant changes.
We understand that many may be feeling a wide range of emotions, including excitement for the Cavallos’ new journey…
…but also feelings of disappointment, frustration, anger, hurt, confusion, overwhelm, helplessness, disillusionment, hopelessness, abandonment, and betrayal.
We want to validate that this reaction is perfectly understandable and we are so sorry you’re feeling this way.
We at PHA are all experiencing bittersweet and conflicting emotions as well.
Individualized integrative holistic patient care has been woven into the fabric of our DNA for over a decade. But as we realized that we were being called to serve in a new way, we came face-to-face with one of our most important values:
Do the hard thing.
Arriving at these decisions was extremely difficult and required months of prayer, soul-searching, and wrestling.
But we know that such change is even harder when you’re the one being left to navigate a new, uncertain reality…faced with new decisions about

whether it’s time for you to move on…or to step into this unconventional, new community that we are building…
As you work through your emotions and think through the decisions that lie ahead, please know that it has always been our practice and intention to create a nurturing environment where our patients have felt embraced, uplifted, valued, and cared for.

FAQs

What kind of appointments will Dr. Cavallo be doing in 2025?
Dr. Cavallo will only be performing surgeries, office procedures, and surgical consultations.
Aside from that, ALL gynecological services will be delivered by Sara Arbogast, NP. Dr. Cavallo will be engaging with Acts2Thrive Members regularly in group settings. (More details on the Membership coming soon. Click here to sign up for updates.)
Do we have any resources or recommendations for those who want to prevent endometriosis from worsening or recurring?
If you have a diagnosis of endometriosis (including a history of endometriosis), you may schedule for our final two educational endometriosis group visits:

Endometriosis Group Visit #1: Led by Dr. Cavallo- Tuesday, 12/4 @ 4pm

Endometriosis Group Visit #2 (“Endo & Nutrition”): Led by nurse practitioner Andrea Borders- Thursday, 12/12 @ 9am.

Given our passion and extensive experience providing endometriosis and infertility care—beyond these final two group visits, the Acts2Thrive Community Membership is designed to help support women in these areas on an ongoing basis in 2025.
Can I still get my refills in 2025? Do I need to be a member?
Medication management is a component of treatment and ongoing management, which is a service only accessible to members.
If you are still unsure of whether or not you will become a member, you may want to contact your primary care physician to see if they are able to manage your medications or if you would need to find a new provider.
Will my insurance cover my medications?
Once our insurance contracts are terminated, all of our providers will be considered “out of network”. Whether or not your insurance covers medication prescribed by an out-of-network provider depends on your particular health plan.
We highly encourage you to contact your insurance for detailed information about your out-of-network benefits. If you have out-of-network benefits, you may be able to submit a claim yourself for full or partial reimbursement.

I’m currently pregnant and have already been accepted for prenatal care or delivery. What does this mean for me?
Dr. Cavallo will still be managing the care of those he has already accepted for delivery and prenatal care even if you decide not to move forward with the membership.
For those who have not delivered by the end of 2024, we will bill insurance for a global bill of all the prenatal care you have received up to that point. All care delivered 1/1/25 and after will be fee-for-service once we have reached your insurance's specific termination date. (Click here for more details)
Do I need to be a member to continue getting my annual exams at PHA in 2025?
Yes. Scheduling annuals as well as all ongoing medical management is only available to Acts 2 Thrive Members.
What is the cost of your out-of-pocket medical services?
We are still working on our 2025 fee schedule, but will update this webpage as soon as we have those details ironed out. Subscribe to FAQ updates here.
